Transition Planning (Ages 14-22)
Transition planning helps students prepare for life after high school. Beginning at age 14, the IEP Team works with students and families to plan for the future based on the student’s strengths, interests, preferences, and goals.
Students are encouraged to actively participate in their IEP meetings and take an active role in planning for their future, with support from families and school staff.

College and Career Planning
In alignment with Massachusetts Department of Elementary and Secondary Education (DESE) guidance, transition planning includes age-appropriate college and career exploration to support informed decision-making and goal-setting.
Postsecondary Planning
Transition planning may include preparation for:
- Postsecondary education
- Vocational training or career pathways
- Competitive, integrated employment
- Independent living and community participation
