The forms below support a variety of employee requests, including leave, reimbursements, additional compensation, and employee information updates. Select a category to view guidance and access the appropriate forms.

Can't find the form you're looking for? Contact the department responsible for the related program, process, or service. Taunton Public Schools is actively reviewing and updating employee forms and resources, and some documents may not yet be available on the website.

  • Find the right form for the type of leave you need


    If you are: attending a conference or professional development session

    Use: the Professional Leave Request Form


    If you are: 

    • Requesting personal leave for the day before or after the December, February, or April Vacation
    • Requesting more than two consecutive days
    • Requesting leave during the last week of school
    • Requesting a date that is full in Frontline

    Use: the Personal Leave Request Form


    If you are: 

    • Requesting extended medical leave
    • Taking leave following the birth or adoption of a child
    • Caring for an immediate family member with a serious health condition

    Use: the Family Medical Leave Act (FMLA) form

    Staff must complete the district FMLA form and the federal certification form that matches the type of leave requested.


    If you are: 

    • Requesting bereavement leave beyond the days included in your contract
    • Requesting bereavement leave for someone not listed in your contract language
    • Requesting non-consecutive bereavement leave days

  • Use the forms below to request reimbursement for approved coursework, professional development activities, mileage, and other eligible work-related expenses.

    Before submitting a reimbursement request, employees should review the In-District Mileage Chart and the City of Taunton Travel Ordinance, which outlines reimbursement rates, per diem allowances, and other travel-related requirements.

    If you are seeking reimbursement foran approved college or university course

    Use: the Course Approval Reimbursement Form


    If you are seeking reimbursement for: approved professional development activities and related eligible expenses, including registration fees, mileage, lodging, and meals when applicable

    Use: the Professional Development Reimbursement Form


    If you are seeking reimbursement for: approved work-related expenses not associated with coursework or professional development, such as materials, supplies, or other authorized purchases made on behalf of the district

    Use: the Expense Reimbursement Form


    If you are seeking reimbursement for: approved mileage incurred while conducting district business and not as part of a professional development activity

    Mileage reimbursement requests for in-district travel should be calculated using the In-District Mileage Chart.

    If this is your first reimbursement request and you have not previously submitted tax documentation, please complete a W-9 Form

  • Use the forms below to request compensation for approved work performed outside of your regular duties or contract hours.

    If you: have completed approved curriculum writing or curriculum revision work

    Use: the Curriculum Revision-Writing Pay Form


    If you: created lesson plans for short or long-term educator absence and are eligible for additional compensation

    Use: the Coverage Planning Pay Form


    If you: presented or facilitated an approved in-house professional development session

    Use: the Presenter’s Payroll Form


    If you: attended an approved in-house professional development activity outside your contractual hours

    Use: the Professional Development Pay Form


    If you: provided approved tutoring services before or after school hours

    Use: the Tutoring Before and After School Pay Form

  • If you: need to update your legal name or address on file with the district

    Use: the Change of Name/Address form

  • Employees requesting a wage adjustment based on additional credits or an earned degree should complete the appropriate Wage Adjustment Request Form for their bargaining unit.


    TEA Members
    If you are a member of the Taunton Education Association (TEA) and are requesting a wage adjustment for additional credits or an earned degree

    Use: The TEA Wage Adjustment Request Form.

    TEA wage adjustments are processed twice each year. Completed requests and official transcripts must be submitted by:

    • September 15 for the October 1 wage adjustment cycle
    • January 15 for the February 1 wage adjustment cycle

    TAA Members
    If you are a member of the Taunton Administrators Association (TAA) and are requesting a wage adjustment for additional credits or an earned degree

    Use: The TAA Wage Adjustment Request Form.

    TAA wage adjustments take effect after you submit the required documentation. Transcripts for the current school year must be submitted by April 15. Requests submitted after April 15 will be processed for salary adjustment the following September.


    Transcript Requirement
    Wage adjustment requests must include an official paper transcript in a sealed envelope documenting the completed coursework. Electronic transcripts are not accepted.

Each year, the district honors employees who demonstrate dedication, leadership, positivity, and a strong commitment to supporting student success. Nominations are generally accepted during the month of April, and winners are announced at Convocation at the start of the school year.
